Home Care Packages (HCP) are a vital resource for older Australians seeking affordable care services at home, especially for those with complex needs beyond the Commonwealth Home Support Programme.
These packages offer the option for funding to be allocated specifically to self funded nutrition and dietitian services, ensuring seniors have access to essential dietary support and expertise.
Our dietitian’s may support seniors eligible under Home Care Package funding via Clinic consultations, Telehealth and Home Visits. We understand that mobility and transportation can be challenging for seniors, which is why we offer flexible scheduling and personalized care in the comfort of your own home at the highest quality.
